Through funding made possible by Saskatchewan Seniors Mechanism, the Town of Shaunavon has entered into an agreement to be a pilot initiative for Social Prescribing for older adults. This 3.5 year project funds a half-time contract position to implement social prescribing for older adults in the community.
Job Summary:
The Community Connector accepts referrals from health care providers or other sources to connect one-on-one with older adults to empower them to take control of their health and wellbeing through a holistic approach by linking them with community groups and services for practical and emotional support.
Experience:
Preference may be given to individuals who have worked with an aging population. Experience working with individual case management/care planning will be considered an asset.
Qualifications:
Experience in project/program management
Ability to listen and empathize with individuals
Commitment to reducing health inequalities
Demonstrates personal accountability and considerable self-initiative
Ability to follow and create policies and procedures e.g. confidentiality
Basic computer knowledge and experience with data collection
Commitment to personal and professional development
Knowledge of the local community is considered an asset
Access to own transportation, valid class 5 drivers license
Able to work independently and with different members of the community
Clear understanding of advocacy
Requirement to provide Criminal record check/Vulnerable Person Check from within the last 6 months. Willingness to work flexible hours in order to meet work demands. This will be a home-based position with access to office and meeting space as necessary.
