At this time, The Beaches Nurse Next Door is looking for skilled and qualified Health Car Planners who are organized, service oriented, empatheitc and compassionate, knowledgeable and detail oriented. Health Care Planners will require nursing background and be responsible for developing long and short-term medical care strategies for patients with medical conditions, ensuring coordinated care among medical providers, caregivers and family members. Reviewing patient diagnoses and conferring with medical providers to understand the extent of the patient's condition. Excellent research skills are also vital to develop comprehensive care plans. The ability to coordinate efficiently and effectively with other members is an integral part of the role.
